IMPORTANT – READ NOW

 In an effort to minimize credit card fraud, all VISA, MasterCard, Diners Club, Discover, and JCB orders will require a signed and imprinted credit card voucher. 

If you are using an American Express card, you may simply fax a copy of your credit card (front and back) to us. 

To ensure that your phone is activated during your rental term, please do the following:

1.                 Place your credit card; face up, on a flat surface, with the credit card voucher on top of your credit card.

2.                Using a crayon or a pencil placed on its side, rub lightly over the voucher.  The credit card information will appear on the voucher.

3.                Write on the voucher either your rental order number if you know it, or the “bill to” name for your phone rental. 

3.                 Sign the voucher and fax it to 415-346-1080. This must be done ASAP to ensure your phone is activated.
